PharmServ has a Direct - Hire opportunity with a closed-door long-term care pharmacy 55miles South of Bloomington, MN. This pharmacy is part of a family-owned and locally operated business with deep roots in Minnesota. They are dedicated to supporting residents of nursing homes, assisted living, and rehabilitation facilities.  

Pharmacists in this role are responsible for managing medication therapy, reviewing prescriptions for safety and effectiveness, and collaborating with healthcare providers to optimize patient care. The pharmacy values a knowledgeable, hands-on team environment, giving staff the tools and support to deliver accurate and attentive pharmaceutical care.

Facility Duties and Expectations:  

• Manage and review medication orders for residents in long-term care facilities, ensuring proper dosing, interactions, and adherence to therapy protocols. 
• Collaborate with nurses and healthcare providers to optimize medication regimens and provide clinical guidance. 
• Maintain accurate records of all prescriptions and ensure compliance with state and federal regulations. 
• Supervise pharmacy technicians 
• Participate in quality improvement initiatives and continuous pharmacy process enhancements. 
• Respond to on-call needs during scheduled rotations, providing timely support to facilities and staff. 
• Communicate regularly with pharmacy management regarding operational updates, patient care, and regulatory compliance. 

Schedule:  

• 4 days a week 
• 35 – 40hr per week 
• Varying hours between 7:45a – 6p 
• Rotating weekend every 7th weekend 
• On-Call 1 Night every other week, and the weekend you work 
    

Required Qualifications:  

• A Doctor of Pharmacy (Pharm.D.) or Bachelor of Pharmacy (B.S. Pharm.) degree from an accredited institution 
• Valid, active pharmacist license in Minnesota in good standing with the Minnesota Board of Pharmacy 

Preferred Qualifications: 

• Experience in LTC setting 
• Ability to handle high stress environments
