Long-Term Care Pharmacist

PharmServ has a Direct-Hire full-time opportunity with a trusted long-term care pharmacy partner 105 miles northwest of Rochester, MN. Pharmacists here manage medications from start to finish, review prescriptions for safety, coordinate directly with physicians and care staff, and help residents get the right treatment safely and on time.

Compensation for this Position:  

• Wage – Up to $133k/yr

Facility Duties and Expectations:  

• Double-check and prepare medications to ensure residents receive what they need 
• Adjust and oversee medication plans based on how residents respond and recover 
• Partner with doctors, nurses, and care staff to fine-tune treatment approaches 
• Assess prescriptions for safety concerns, including interactions and allergies 
• Contribute to process improvements, formulary decisions, and compliance efforts 
• Keep organized, accurate records that meet all regulatory standards 

Pharmacy Hours of Operation:  

• Monday – Friday: 8:30a – 6:30p 
• Saturday – Sunday: 9a – 1p 
• 24hr On-Call 
  

Required Qualifications: 

• A Doctor of Pharmacy (Pharm.D.) or Bachelor of Pharmacy (B.S. Pharm.) degree from an accredited institution   
• Valid, active pharmacist license in Minnesota in good standing with the Minnesota Board of Pharmacy
